Indications

Pharmacology

  • Inhibits platelet activation
  • Inhibits platelet aggregation
  • Irreversible binding to P2Y12 class of ADP receptors
  • Dose-dependent inhibition of platelet aggregation
  • Inhibition of platelet cyclooxygenase
  • Inhibition of thromboxane A2 generation
  • Rapid absorption after oral administration
  • Metabolized by the liver
  • Excretion in urine and faeces

Dosage & Administration

  • Recommended dose of 75 mg once daily
  • Single 300 mg loading dose for acute coronary syndrome
  • Initiation with or without food
  • Combination with Aspirin
  • Initiation with Heparin

Interaction

  • Clopidogrel-mediated inhibition of ADP-induced platelet aggregation
  • Potentiation of aspirin effect
  • No modification of heparin dose or effect
  • Increased occult gastrointestinal blood loss with NSAIDs
  • Caution in co-administration with warfarin

Contraindications

  • Hypersensitivity to the drug substance
  • Active pathological bleeding

Side Effects

  • Generally well tolerated

Pregnancy & Lactation

  • Use during first and second trimesters if clearly needed
  • Contraindicated during the third trimester
  • Discontinuation during breastfeeding

Precautions & Warnings

  • Prolongs the bleeding time
  • Rare reports of Thrombotic thrombocytopenic purpura (TTP)
  • Potential for Reye's syndrome
  • Risk of hypersensitivity reactions

Use in Special Populations

  • Not recommended for children under 12 years
  • Potential contributory factor in Reye's syndrome
