Indications

  • Treatment of upper respiratory tract infections (URTI)
  • Urinary tract infections
  • Bactericidal activity against Gram-positive and Gram-negative organisms
  • Treatment of lower respiratory tract infections
  • Clinical efficacy against commonly occurring pathogens
  • High stability in the presence of beta-lactamase enzymes

Pharmacology

  • Third generation cephalosporin antibiotic
  • Orally active
  • Bactericidal against broad spectrum of bacteria
  • Interferes in the synthesis of bacterial cell wall
  • Highly stable in the presence of beta-lactamase enzyme
  • About 40% to 50% absorption

Dosage & Administration

  • Usual course of treatment is 7 days
  • Recommended adult dosage of 200-400 mg daily
  • Dosage for children based on age and weight
  • Dosage adjustment in severe renal impairment
  • Recommended dosage in typhoid
  • Special dosage for children less than 6 months

Interaction

  • Increases in prothrombin times noted in some patients
  • Caution required in patients receiving anticoagulant therapy

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Mild and self-limiting adverse reactions
  • Gastrointestinal disturbances: diarrhea, nausea, abdominal pain
  • Central nervous system effects: headache, dizziness
  • Hypersensitivity reactions: rash, pruritis, urticaria
  • Hematological and clinical chemistry effects: thrombocytopenia, leukopenia
  • Other possible reactions: genital pruritis, vaginitis

Pregnancy & Lactation

  • Caution advised during pregnancy
  • Use during lactation requires caution

Precautions & Warnings

  • Caution in patients with hypersensitivity to other drugs
  • Use with caution in penicillin-sensitive patients
  • Caution in patients with markedly impaired renal function
  • Alteration of normal flora of the colon may permit overgrowth of clostridia

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • Not significantly removed by dialysis
