Indications
- Reduction of Myocardial Infarction (MI) and Stroke in patients with non-ST-segment elevation Acute Coronary Syndrome (ACS)
- Reduction of Myocardial Infarction (MI) and Stroke in patients with ST-segment elevation Acute Coronary Syndrome (ACS)
- Reduction of MI and Stroke in patients with recent MI, recent Stroke, or established Peripheral Arterial Disease
Pharmacology
- Clopidogrel is a prodrug
- Inhibits platelet activation and aggregation
- Irreversibly binds to P2Y12 class of ADP receptors on platelets
- Aspirin inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase
- Inhibits the generation of thromboxane A2
Pharmacokinetics
- Plasma concentrations of the parent compound are very low and generally below quantification limit
- Clopidogrel is extensively metabolized by the liver
- Approximately 50% excreted in urine and 46% in faeces
- Elimination half-life of main circulating metabolite is 8 hours
Dosage & Administration
- Recommended dose: 75 mg once daily with or without food
- No dosage adjustment for elderly patients or patients with renal impairment
- Patients with non-ST-segment elevation ACS: initiate with a single 300 mg loading dose
- Aspirin (75 mg-325 mg once daily) should be initiated and continued in combination with Clopidogrel
- Most patients with acute coronary syndrome also received heparin acutely
Interactions
- No modification of Clopidogrel-mediated inhibition by Aspirin
- Clopidogrel potentiates the effect of aspirin on collagen-induced platelet aggregation
- Clopidogrel does not alter the effect of heparin on coagulation
- NSAIDs and Clopidogrel should be co-administered with caution
- Caution with concomitant administration of Clopidogrel with warfarin
Contraindications
- Hypersensitivity to the drug substance or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Use during first and second trimesters only if clearly needed
- Contraindicated during the third trimester
- Unknown excretion of Clopidogrel in human breast milk
- Aspirin known to be excreted in human milk
- Discontinue drug during breastfeeding
Precautions & Warnings
- May prolong bleeding time
- Rare reports of Thrombotic thrombocytopenic purpura (TTP) following use
- Reye's syndrome may develop in individuals with chicken pox, influenza or flu symptoms
- Hypersensitivity reactions reported in patients receiving Replet Plus or history of hypersensitivity to other thienopyridines
Use in Special Populations
- Not recommended for children, particularly those under 12 years
- Aspirin may be a contributory factor in the causation of Reye’s syndrome in some children
Overdose Effects
- Clopidogrel overdose may lead to bleeding complications
- Moderate aspirin intoxication may cause dizziness, headache, tinnitus, confusion, and gastrointestinal symptoms
- Severe aspirin intoxication may lead to respiratory alkalosis, respiratory acidosis, metabolic acidosis, hyperthermia, perspiration, dehydration
- Treatment with platelet transfusion, induction of vomiting, gastric lavage, haemodialysis, and other symptomatic treatments
