Indications
- Broad-spectrum bactericidal activity
- Treatment of Upper Respiratory Tract Infections (URTI)
- Treatment of Lower Respiratory Tract Infections
- Treatment of Urinary Tract Infections (UTI)
- Effective against Streptococcus pneumoniae
- Effective against Escherichia coli
- High stability in the presence of beta-lactamase enzymes
Pharmacology
- Third generation semisynthetic cephalosporin antibiotic
- Bactericidal against broad spectrum of gram positive and gram negative bacteria
- Interference in bacterial cell wall synthesis
- High stability in the presence of Beta-lactamase enzyme
- About 40-50% absorption with or without food
Dosage & Administration
- Usual course of treatment is 7 days, can be continued for up to 14 days if required
- Recommended adult dosage 200-400 mg daily
- Usual dosage for children 8 mg/kg/day
- Safety and efficacy not established in children aged less than 6 months
- Dosage adjustment in severe renal impairment
- Dose and regimen for patients on chronic ambulatory peritoneal dialysis or haemodialysis
Interaction
- Potential increases in prothrombin times in some patients
- Caution in patients receiving anticoagulant therapy
Contraindications
- Known hypersensitivity to cephalosporin antibiotics
Side Effects
- Majority of adverse reactions observed in clinical trials were mild and self limiting
- Frequent side-effects: diarrhoea, stool changes
- Less frequent gastrointestinal side-effects: nausea, abdominal pain, dyspepsia, vomiting, flatulence
- Central nervous system side-effects: headache, dizziness
- Hypersensitivity reactions: rash, pruritis, urticaria, drug fever, arthralgia
- Hematological and clinical chemistry reactions: thrombocytopenia, leukopenia, eosinophilia
- Miscellaneous reactions: genital pruritis, vaginitis
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Caution when administered to a nursing woman
Precautions & Warnings
- Caution in patients with known hypersensitivity to other drugs
- Cautious use in penicillin-sensitive patients
- Administration with caution in patients with markedly impaired renal function
- Risk of antibiotic-associated diarrhoea
- Not recommended for use in pregnancy or nursing mothers unless considered essential by the physician
Overdose Effects
- Gastric Lavage may be indicated
- No specific antidote exists
- Not removed in significant quantities by hemodialysis or peritoneal dialysis
- Adverse reactions at single doses up to 2 g did not differ from profile seen in patients treated at recommended doses
