Indications
- Adjunct to diet and exercise
- Improves glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- DPP-4 inhibitor
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
Dosage & Administration
- Recommended dose: 50 mg twice a day or 100 mg once daily
- Can be taken with or without food
Interaction
- No meaningful alteration of pharmacokinetics with certain drugs
- Slightly increases mean concentration of Digoxin
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions: headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia may occur in certain patients
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly needed
- Excretion in lactating rats
- Caution when administered to nursing women
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment in patients with renal insufficiency
- Use with caution in patients with medications known to cause hypoglycemia

Overdose Effects
- Maximal mean increases in QTc observed at high doses
- Usual supportive measures in event of overdose
- Modestly dialyzable
- Consider prolonged hemodialysis if necessary
