Indications
- Replacement therapy in hypothyroidism
- Suppression of Thyroid Stimulating Hormone (TSH) levels
- Suppression of goitrogenic effects
- Diagnostic aid in suppression tests
Pharmacology
- Contains synthetic Levothyroxine (T4)
- About 30% of T4 is converted to Triiodothyronine (T3)
- T4 is protected by Thyroxine Binding Globulin (TBG)
- Enterohepatic circulation of Thyroid hormones
- Liver is the major site of degradation of Thyroid hormones
Dosage & Administration
- Initial starting dose: 25-50 mcg/day
- Gradual increments in dose at 6-8 week intervals
- Dose adjustments based on age and condition
- Recommended starting dose for newborns and pediatric patients
Interaction
- Concurrent use with tri/tetracyclic antidepressants
- Increased receptor sensitivity to catecholamines
- Addition to antidiabetic or insulin therapy
- Reduction of serum digitalis glycoside levels
Contraindications
- Untreated subclinical or overt Thyrotoxicosis
- Acute Myocardial Infarction
- Uncorrected Adrenal failure
Side Effects
- Adverse reactions associated with hyperthyroidism
- General, CNS, musculoskeletal, cardiovascular, respiratory, gastrointestinal, and dermatologic effects
Pregnancy & Lactation
- Pregnancy Category A
- Possible increase in Levothyroxine requirements during pregnancy
- Excretion in human milk
- Caution during nursing

- Adrenocortical insufficiency in pituitary gland dysfunction
- Careful dosing in cardiovascular disease and severe hypothyroidism
- Need for dosage adjustment with marked change in body weight
- Importance of monitoring T3, T4, and TSH levels
