Indications

Pharmacology

  • Cilnidipine is a dihydropyridine calcium-channel blocker
  • Binds to dihydropyridine binding sites of L-type voltage dependent calcium channel
  • Inhibits Ca2+ influx across cell membranes
  • Causes vasodilation and vascular smooth muscle relaxation
  • Inhibits Ca2+ influx via N-type voltage dependent calcium channels

Dosage and Administration

  • Adult dose: 5-10 mg once daily after breakfast
  • Maximum adult dose: 20 mg once daily
  • Pediatric use safety not established
  • Elderly use: initiate therapy with lowest possible dose (5 mg)

Interactions

  • Interacts with other antihypertensive medications
  • Interacts with antipsychotics causing hypotension
  • Interacts with quinidine, carbamazepine, phenytoin, rifampicin, cimetidine, and erythromycin

Contraindications

  • Contraindicated in patients with known sensitivity to Cilnidipine or excipients
  • Contraindicated in patients with cardiogenic shock, recent MI, acute unstable angina, and severe aortic stenosis

Side Effects

  • Common side effects include dizziness, flushing, headache, hypotension, peripheral oedema, palpitations, GI disturbances, increased micturition frequency, lethargy, eye pain, and depression

Pregnancy and Lactation

  • Not to be administered in pregnant women
  • Avoid administration in nursing mothers
  • Discontinue lactation if administration is indispensable

Precautions and Warnings

  • Administer with care in patients with serious hepatic dysfunction
  • Administer with care in patients with history of serious adverse reactions to calcium antagonists
  • Gradually decrease dosage during discontinuation under close observation
