Indications
- Treatment of essential hypertension in adults
- Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease or type 2 diabetes mellitus with documented target organ damage
Pharmacology
- Non-peptide angiotensin-ll receptor antagonist
- Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin-ll
- Greater affinity for the AT1 receptor than for the AT2 receptor
- Does not inhibit ACE (kininase-ll)
Dosage and Administration
- Usual starting dose of 40mg once a day
- Blood pressure response is dose-related over the range of 20 to 80 mg
- Most antihypertensive effect is apparent within 2 weeks
- Maximal reduction is generally attained after 4 weeks
- No initial dosage adjustment necessary for elderly patients or patients with renal impairment
- May be administered with or without food
Pediatric Use
- Safety and effectiveness not established
Geriatric Use
- No overall differences in effectiveness and safety compared to younger patients
Hepatic Impairment
- Monitor carefully and up titrate slowly in patients with biliary obstructive disorders or hepatic insufficiency
Interactions
- Increased risk of renal impairment and loss of antihypertensive effect when used with NSAIDs
- Co-administration with aliskiren in patients with diabetes should be avoided
Contraindications
- Known hypersensitivity to this product or any of its components
Side Effects
- Sinus pain and congestion (sinusitis)
- Back pain
- Diarrhea
- Intermittent claudication
- Skin ulcer
Pregnancy and Lactation
- Pregnancy Category C (first trimester) and D (second and third trimester)
- Potential adverse effects on the nursing infant
Precautions and Warnings
- Avoid fetal or neonatal exposure
- Hypotension
- Monitor in patients with impaired hepatic or renal function
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
Overdose Effects
- Hypotension
- Dizziness
- Tachycardia
- Bradycardia
- Increase in serum creatinine
- Acute renal failure
