Indications

Pharmacology
- Tranexamic acid as a strong antifibrinolytic action
- Inhibition of plasminogen activation
- Low acute and chronic toxicity
- Well absorbed by oral route
- Excretion mainly by renal route
- No interference with clotting processes
- No tendency to thrombophilia
Dosage & Administration
- Usual dose for adults: 500-1000 mg 3 times daily
- Recommended daily doses for prophylaxis: 0.5-1 gm orally, 500 mg by parenteral route
- Oral dose for therapy of hemorrhagic manifestations: 1-3 gm given in divided doses
- Dosage for children based on body weight
- No reduction in dosage for elderly patients
Interactions
- Incompatible with solutions containing penicillins
- Antagonizes the antifibrinolytic action of thrombolytic drugs
- Potential for thrombus formation increased by estrogen containing drugs
- Direct admixture with whole blood should be avoided during transfusion
Contraindications
- Known individual hypersensitivity to the product
- Thromboembolic disease
- Arterial and venous thrombosis
- Endocavitary hemorrhages
- Serious kidney failure
Side Effects
- Infrequent cases of sense of fatigue, conjunctival irritation, nasal blockage
- Rare cases of postural hypotension
- Nausea, diarrhea, gastric pyrosis after oral administration
- Avoid or suspend treatment in case of hypersensitivity
Pregnancy & Lactation
- Tranexamic Acid should not be administered during known and presumed pregnancy
- Passes into breast milk to a concentration of approximately one hundredth of the concentration in the maternal blood
- Unlikely to have an antifibrinolytic effect in the infant
Precautions & Warnings
- Use in cases of hyperfibrinolysis
- Prophylactic treatment must begin 24 hours before the operation
- Therapy of hemorrhages must be prolonged for at least 24 hours after manifestations have disappeared
- Reduce doses in hematuria to prevent clot formation in the urinary tract
- Caution required in cardiac and hepatic patients
