Indications
- Monotherapy in the treatment of partial seizures for adults
- Adjunctive therapy in the treatment of partial seizures for adults and pediatrics
- Monotherapy in the treatment of partial seizures in children 4-16 years
- Adjunctive therapy in the treatment of partial seizures in children 2-16 years
Pharmacology
- Mechanism of action involving blockade of voltage-sensitive sodium channels
- Exerted primarily through the metabolite derivative (MHD) of Oxcarbazepine
- Stabilization of hyper excited neural membranes
- Inhibition of repetitive neuronal firing
Dosage & Administration
- Recommended initial dose for adults: 600 mg/day given twice a day
- Maximum daily dose for adults: 2400 mg/day
- Dependent pediatric dose based on weight
- For pediatric patients aged 2 to <4 years and under 20 kg, starting dose of 16 to 20 mg/kg/day may be considered
- Specific dosage adjustments for patients with creatinine clearance <30 mL/min
Interaction
- Inhibition of the CYP2C19 enzyme by Oxcarb XR and its metabolite
- Potential interactions with medications metabolized by CYP2C19
- Reduced effectiveness of certain hormonal contraceptives
- Altered AUC and bioavailability of concomitant medications
Contraindications
- Hypersensitivity to the active substance or any of the excipients
Side Effects
- Common adverse reactions: somnolence, headache, dizziness, nausea, fatigue
- Rare occurrence of clinically significant hyponatremia
- Reports of class I hypersensitivity reactions including rash, pruritus, urticaria, and angioedema
Pregnancy & Lactation
- Potential to cause serious birth defects and bleeding disorders during pregnancy
- Administration of vitamin K1 as a preventive measure in the last few weeks of pregnancy
- Excretion of Oxcarbazepine and its active metabolite in human breast milk
Precautions & Warnings
- Caution advised when combining with alcohol due to possible additive sedative effect
- Recommendation for gradual withdrawal to minimize potential increased seizure frequency
