Indications

Pharmacology

  • Inhibits microsomal protein synthesis in susceptible organisms
  • Binds to donor site on 50S subunit of bacterial ribosome
  • Prevents translocation to the donor site
  • Active against most Gram-positive bacteria, Chlamydia, some Gram-negative bacteria and Mycoplasmas

Interaction

  • Contraindicated with HMG-CoA reductase inhibitors
  • Contraindicated with gastroprokinetic agents
  • Contraindicated with ergot alkaloids
  • Contraindicated with antipsychotics
  • Contraindicated with anti-gout agents
  • Not recommended with antiarrhythmics drugs

Contraindications

  • Known hypersensitivity to clarithromycin or erythromycin
  • History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

Precautions & Warnings

  • Avoid in patients receiving drugs known to prolong the QT interval
  • Discontinue if signs and symptoms of hepatitis occur
  • Increased risk of all-cause mortality in patients with coronary artery disease

Use in Special Populations

  • Safety and effectiveness in pediatric patients 6 months and older
  • Safety and effectiveness in pediatric patients 20 months and older with advanced HIV infection

Overdose Effects

  • Ingestion of large amounts can produce gastro-intestinal symptoms
  • Adverse reactions accompanying overdosage should be treated promptly
