Description
- Diablo Tablet is used for the management of type 2 diabetes mellitus in adults, children from 10 years of age, and adolescents. It helps in controlling postprandial plasma glucose, improving insulin sensitivity, and reducing diabetic complications in overweight patients.
Indications
- Treatment of type 2 diabetes mellitus in overweight patients when dietary management and exercise alone does not result in adequate glycaemic control
- Monotherapy or in combination with other oral antidiabetic agents or insulin in adults
- Monotherapy or in combination with insulin in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients treated with metformin as first-line therapy after diet failure
Pharmacology
- Metformin is a biguanide type oral antihyperglycemic drug used in the management of type 2 diabetes
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production, intestinal absorption of glucose, and improves insulin sensitivity
Dosage & Administration
- Dosage must be individualized based on effectiveness and tolerance, not exceeding the maximum recommended daily doses
- Adult starting dose: 500 mg twice a day or 850 mg once a day, dosage increases made in increments of 500 mg weekly or 850 mg every 2 weeks, up to a total of 2000 mg per day
- Children starting dose: 500 mg twice a day, dosage increases made in increments of 500 mg weekly up to a maximum of 2000 mg per day
- Dosage of Metformin XR for adults: 500 mg once daily with the evening meal, dose increased in increments of 500 mg weekly, up to a maximum of 2000 mg once daily with the evening meal
Interaction
- No information available about chronic co-administration with Furosemide
- Nifedipine may enhance the absorption of Diablo
- Cationic drugs have the potential for interaction with Diablo by competing for common renal tubular transport systems
- Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Acute metabolic acidosis, severe renal failure, acute conditions altering renal function, conditions causing tissue hypoxia
- Acute or chronic diseases causing tissue hypoxia, dehydration, severe infection, shock
Side Effects
- Hemolytic anemia
- Lactic acidosis (rare)
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance, encephalopathy
- Gastrointestinal disorders such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite
- Liver function test abnormalities or hepatitis (very rare)
- Skin reactions such as erythema, pruritus, urticaria (very rare)
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality. Insulin is recommended during pregnancy
- Metformin excreted into human breast milk, breastfeeding not recommended during metformin treatment
Precautions & Warnings
- Substantial excretion by the kidney, risk of accumulation and lactic acidosis increases with impaired renal function
- May lower vitamin B12 level, increases risk of hypoglycemia when used in combination with insulin or insulin secretagogue
- Regular monitoring of renal function advised for all diabetics, metformin therapy should be stopped before surgery and reinstated only after renal function is regained
Use in Special Populations
- Elderly subjects: potential for decreased renal function, dosage adjustment necessary based on renal function
- Diagnosis of type 2 diabetes mellitus should be confirmed before treatment with metformin is initiated, no long-term data on effect on growth and puberty available
- Particular caution recommended when prescribing to children aged between 10 and 12 years
- Creatinine clearance should be determined before treatment initiation, GFR assessment before and regularly during treatment
- Contraindicated in patients with GFR <30 ml/min
Overdose Effects
- Hypoglycemia not seen with Diablo doses up to 85 gm, lactic acidosis has occurred in such circumstances
- Lactic acidosis is a medical emergency and must be treated in hospital
- Hemodialysis is the most effective method to remove lactate and Diablo
