Indications

• Acute treatment of schizophrenia
• Maintenance treatment of schizophrenia
• Treatment of related psychoses
• Treatment of positive symptoms in schizophrenia
• Treatment of negative symptoms in schizophrenia
• Treatment of acute manic episodes in bipolar disorder
• Treatment of mixed episodes in bipolar disorder
• Treatment of bipolar disorder with psychotic features
• Treatment of bipolar disorder with rapid cycling course

Pharmacology

• Affinities for serotonin 5HT2A/2C, 5HT3, 5HT6 receptors
• Affinities for dopamine D1, D2, D3, D4, D5 receptors
• Affinities for cholinergic muscarinic receptors M1-M5
• Affinities for α1 adrenergic receptors
• Affinities for histamine H1 receptors
• Unknown mechanism of action in schizophrenia
• Proposed dopamine and serotonin type 2 (5HT2) antagonism

Dosage

• Initial dose of 5-10 mg once daily for schizophrenia and bipolar disorder
• Daily dosage range of 5-20 mg
• Maximum dosage of 20 mg daily for schizophrenia and bipolar disorder
• Initial dose of 15 mg once daily for monotherapy of mania

Administration

• Can be given without regard to meals
• Gradual tapering of the dose while discontinuing

Interaction

• Antagonizes effects of levodopa and dopamine agonists
• May be affected by CYP1A2 inducers like Omeprazole and Rifampicin
• May be inhibited by CYP1A2 inhibitors
• Affected by carbamazepine
• Reduced oral bioavailability by activated charcoal
• Caution with centrally acting drugs and alcohol

Contraindications

• Known hypersensitivity to any ingredient
• Known risk for narrow-angle glaucoma

• Very common: somnolence
• Very common: weight gain
• Increased appetite
• Elevated glucose levels
• Elevated triglyceride levels
• Dizziness
• Akathisia
• Parkinson's disease symptoms
• Dyskinesia
• Orthostatic hypotension
• Mild and transient anticholinergic effects
• Constipation
• Dry mouth
• Asthenia
• Edema
• Photosensitivity reaction

Pregnancy & Lactation

• Use in pregnancy if potential benefits justify potential risk
• Patients advised to notify physician if pregnant
• No reports to show teratogenicity
• Breastfeeding not recommended

Precautions & Warnings

• Use cautiously in patients with history of seizures
• Use cautiously in patients with low leukocyte and/or neutrophil counts
• Use cautiously in conditions associated with seizures
• Use cautiously in drug-induced bone marrow depression/toxicity
• Use cautiously in hypereosinophilic conditions
• Use cautiously in impaired hepatic function
• Use cautiously in patients with hepatotoxic medicine
• Use cautiously in patients using drugs increasing QT interval
• Caution about operating hazardous machinery

Use in Special Populations

• Not studied in subjects under 18 years of age
• Starting dose of 5 mg/day for elderly patients
• Starting dose of 5 mg/day for patients with hepatic impairment
• Starting dose of 5 mg/day for patients with renal impairment

Therapeutic Class
