Indications
- Treatment of chronic lymphocytic leukemia (CLL) in adults
- Treatment of CLL with 17p deletion or TP53 mutation
- Treatment of CLL after chemoimmunotherapy and B-cell receptor pathway inhibitor failure
Pharmacology
- Selective BCL-2 inhibitor
- Orally bioavailable small-molecule
- Restores apoptosis process
- Cytotoxic activity in tumor cells
Dosage & Administration
- Starting dose of 20 mg once daily
- Gradual increase to 400 mg daily over 5 weeks
- 400 mg daily in combination with Rituximab for 24 months
- Treatment continuation until disease progression or intolerance
Interaction
- Contraindicated with strong CYP3A inhibitors
- Avoid concomitant use with moderate CYP3A inhibitors
- Caution with P-gp and BCRP inhibitors
- Contraindicated with strong CYP3A inducers
Contraindications
- Hypersensitivity to active substance or excipients
- Concomitant use of strong CYP3A inhibitors
- Concomitant use of St. John's wort preparations
Side Effects
- Neutropenia

- Upper respiratory tract infection
- Pneumonia
- Febrile neutropenia
Pregnancy & Lactation
- Use of highly effective contraception during treatment
- Unknown effect on hormonal contraceptives
- Embryo-foetal toxicity demonstrated in animal studies

- Tumour lysis syndrome risk
- Neutropenia monitoring and management
- Avoid live attenuated vaccines
- Caution with CYP3A4 inducers
