Medicine Details

Indications

  • Replacement therapy in hypothyroidism of any etiology
  • Suppression of Thyroid Stimulating Hormone (TSH) levels in presence of goitres and nodules
  • Suppression of goitrogenic effects of other drugs
  • Diagnostic aid in suppression tests

Pharmacology

  • Contains synthetic Levothyroxine (T4)
  • Identical to natural hormone T4 produced in Thyroid gland
  • About 30% of T4 is converted to more active Triiodothyronine (T3)
  • TBG (Thyroxine Binding Globulin) is major carrier of T4
  • Half-life of elimination of T4 is 6 to 7 days
  • Liver is major site of degradation of Thyroid hormones
  • Enterohepatic circulation of the Thyroid hormones

Dosage & Administration

  • Initial starting dose: 25-50 mcg/day
  • Gradual increments in dose at 6-8 week intervals
  • Severe hypothyroidism: Initial dose is 12.5-25 mcg/day
  • Pediatric Dosage (Newborns): 10-15 mcg/kg/day
  • Pediatric Dosage (Infants and Children): Varies with age
  • Dose adjustment based on clinical response and laboratory parameters

Interaction

  • Concurrent use of tri/tetracyclic antidepressants may increase therapeutic and toxic effects
  • Addition of Thyronor to antidiabetic or insulin therapy may result in increased requirements

Contraindications

  • Untreated subclinical or overt Thyrotoxicosis
  • Acute Myocardial Infarction
  • Uncorrected Adrenal failure

Side Effects

  • Adverse reactions associated with Thyronor therapy
  • Primarily those of hyperthyroidism due to therapeutic overdose
  • Includes symptoms in multiple body systems

Pregnancy & Lactation

  • Pregnancy Category A
  • Levothyroxine requirements may increase during pregnancy
  • Caution recommended during nursing

Precautions & Warnings

  • Adrenocortical insufficiency may be present in patients with hypothyroidism due to pituitary gland function decrease
  • Great care needed in patients with cardiovascular disease and/or severe and long-existing hypothyroidism
  • Dosage adjustment required with marked change of body weight
  • Monitoring of blood levels of T3 and T4

Overdose Effects

  • Signs and symptoms of overdose are those of hyperthyroidism
  • Treatment is symptomatic
