Indications
- Symptomatic treatment of chronic constipation in adults in whom laxatives fail to provide adequate relief
Pharmacology
- Selective stimulator of the 5-HT4 receptors
- Acts on smooth muscle cells, enterochromaffin cells, and myenteric plexus
- Stimulates motility by interacting specifically with 5-HT4 receptors in the GI tract
- Causes release of acetylcholine and further contraction of the muscle layer of the colon
- Relaxes the circular muscle layer leading to propulsion of luminal content
Dosage & Administration
- Adults: 2 mg once daily with or without food
- Older people: Start with 1 mg once daily, can be increased to 2 mg once daily
- Not recommended for children and adolescents younger than 18 years
Interaction
- Low interaction potential and therapeutic concentrations are not expected to affect the CYP-mediated metabolism of co-medicated medicinal products
- Effect of potent inhibitors on systemic exposure to prucalopride
- No clinically relevant effects on the pharmacokinetics of certain drugs
Contraindications
- Hypersensitivity to the active substance or excipients
- Renal impairment requiring dialysis
Side Effects
- Most frequently reported adverse reactions include headache and gastrointestinal symptoms such as abdominal pain, nausea, and diarrhea
- Adverse reactions occur predominantly at the start of therapy and usually disappear within a few days
Pregnancy & Lactation
- Not recommended during pregnancy
- Use effective contraception during treatment
- Animal studies do not indicate direct or indirect harmful effects with respect to pregnancy
- Not recommended during breastfeeding
Precautions & Warnings
- Renal excretion is the main route of elimination of prucalopride
- Caution in severe hepatic impairment
- Possible reduction in efficacy of oral contraceptives during severe diarrhea
- Contains lactose (not suitable for patients with rare hereditary problems of galactose intolerance, Lapp lactase deficiency, or glucose-galactose malabsorption)
Use in Special Populations
- Dose adjustment for patients with severe renal impairment
- Caution in prescribing to patients with severe hepatic impairment
- No dose adjustment required for patients with mild to moderate renal or hepatic impairment
Overdose Effects
- Symptoms include headache, nausea, and diarrhea
- Specific treatment not available
- Supportive and symptomatic measures may be required
- Correction of electrolyte disturbances may be necessary in case of extensive fluid loss
