Indications

  • Treatment of hypertension

Pharmacology

  • Selectively blocking the AT1 receptor
  • Independent of the pathway for angiotensin II synthesis
  • Inhibition of angiotensin II effects
  • Absorption in gastrointestinal tract
  • Plasma concentration peak reached within 1.5 to 3 hours
  • High protein binding
  • Metabolism to primary metabolites
  • CYP2C9 enzyme responsible for metabolism

Dosage & Administration

  • Recommended adult dose of 80 mg orally once daily
  • Starting dose of 40 mg for patients on high doses of diuretics
  • Possible combination with other antihypertensive agents for additional blood pressure reduction

Interaction

  • No observed drug interactions with specific medications
  • Potential attenuation of antihypertensive effect by non-steroidal anti-inflammatory drugs

Contraindications

  • Contraindicated to co-administer Aliskiren with Azilsartan in patients with diabetes

Side Effects

  • Common adverse reaction: diarrhea
  • Other side effects: nausea, asthenia, fatigue, muscle spasm, dizziness, cough

Pregnancy & Lactation

  • Pregnancy Category D
  • Increased risk to the fetus if administered in second or third trimester
  • Potential excretion in human milk
  • Consideration for discontinuation of nursing or drug

Precautions & Warnings

  • Reduced fetal renal function and increased fetal and neonatal morbidity and death in second and third trimesters
  • Risk of symptomatic hypotension in intravascularly volume-depleted patients
  • Reported changes in renal function including renal failure in renal impaired patients

Use in Special Populations

  • Neonatal considerations with in utero exposure to azilsartan
  • Support for blood pressure and renal function if oliguria or hypotension occurs
  • No established safety and effectiveness in pediatric patients under 18 years of age
  • No dose adjustment in elderly patients
  • No dose adjustment in patients with mild-to-severe renal impairment or end-stage renal disease
  • Possible abnormally high serum creatinine values in elderly patients
  • No dose adjustment in mild or moderate hepatic impairment

Overdose Effects

  • Limited data available related to overdose in humans
  • Supportive therapy recommended
  • Not dialyzable
