Indications
- Management of hypertension
- Adjunctive therapy in edema associated with congestive heart failure
- Adjunctive therapy in edema associated with hepatic cirrhosis
- Adjunctive therapy in edema associated with corticosteroid and estrogen therapy
Pharmacology

- Treatment of hypertension
- Adjunctive therapy of edema associated with renal disease
- Adjunctive therapy of edema associated with ascites due to cirrhosis of the liver
- Inhibition of reabsorption of sodium and chloride in the distal renal tubule
- Promotion of water loss
- Long elimination half-life
- Causing increased potassium loss
- Volume reduction for lowering blood pressure
- Reduction of peripheral resistance
- Excretion by the kidneys with an elimination half-life averaging 50 hours
- Minor metabolism and hepatic excretion into the bile
- Excretion in the urine and feces mainly in an unchanged form
Dosage & Administration
- Therapy initiation with the lowest possible dose
- Titration for maximum therapeutic benefit and minimum side effects
- Single dose daily or every other day recommended with food
- Usual adult dose of 25 to 50 mg daily for hypertension
- Clinically useful dosage range of 12.5 to 50 mg daily for hypertension
- Lowest effective dose for edema to be identified by titration
- Maintenance doses not to exceed 50 mg/day for edema
- Adapted dosage to clinical picture and patient response for edema
- Suitable drug for treating hypertension in the elderly
- Dose of 50 mg daily or less recommended for the elderly
Interaction
- Caution in patients with impaired hepatic function
- Caution in postsympathectomy patients
- Cautious dosage schedule in patients with severe coronary or cerebral ateriosderosis
- Potentiation of action of antihypertensive agents
- Increased likelihood of digitalis-induced cardiac arrhythmias
- Hypokalemic effects increased by corticosteroids
- Necessity to adjust the dosage of insulin or oral antidiabetic agents
- Concomitant administration of NSAIDs weakening diuretic and antihypertensive activity
- Increase in responsiveness to curare derivatives and ganglionic blocking agents
- Increase in the incidence of hypersensitivity reactions to allopurinol
- Increase in the risk of adverse effects from amantadine
- Reduction of renal excretion of cytotoxic agents and enhancement of myelosuppressive effects by antineoplastic agents
- Bioavailability increase of thiazide-type diuretics by anticholinergic agents
- Decrease in absorption of thiazide diuretics by cholestyramine
- Decrease in urinary excretion of calcium by thiazide diuretics
- Increase in risk of hyperuricemia and gout-type complications by simultaneous treatment with diuretics
- Increase in tubular calcium reabsorption by concomitant use of thiazide-type diuretics and calcium salts
- Enhancement of hyperglycemic effect of diazoxide by thiazide diuretics
Contraindications
- Anuria
- Severe renal failure
- Severe hepatic failure
- Refractory hypokalemia or conditions involving enhanced potassium loss
- Hyponatremia
- Hypercalcemia
- Symptomatic hyperuricemia
- Hypersensitivity or suspected hypersensitivity to Chlorthalidone and other sulfonamide derivatives or their excipients
- Use with caution in patients with renal disease or impaired hepatic function

- Avoidance of use in pregnant women for hypertension treatment
- Avoidance of use in pregnancy for other indications in the first trimester
- Avoidance of use in nursing mothers due to presence in breast milk
Precautions & Warnings
- Dosage reduction in moderate renal failure
- Avoidance in advanced renal failure
- Risk of precipitating hepatic encephalopathy in patients with liver cirrhosis and ascites
- Avoidance in pregnancy
- Avoidance in lactating mothers due to presence in breast milk
Overdose Effects
- Symptoms of acute overdosage include nausea, weakness, dizziness, and disturbances of electrolyte balance
- No specific antidote recommended
- Gastric lavage followed by supportive treatment recommended
