Indications

  • Mild to moderate infections
  • Adults and adolescents (12 years of age or older)
  • Bacterial exacerbation of chronic bronchitis
  • Community acquired pneumonia
  • Pharyngitis
  • Tonsillitis
  • Uncomplicated skin and skin-structure infections

Pharmacology

  • Preparation of Cefditoren Pivoxil
  • Broad spectrum third generation cephalosporin antibiotic
  • Prodrug hydrolyzed by esterases
  • Antibacterial activity against gram-positive and gram-negative pathogens
  • Inhibition of cell wall synthesis
  • Stable in the presence of various ß-lactamases

Dosage & Administration

  • Taken after meals
  • Community-acquired pneumonia: 400 mg twice daily for 14 days
  • Acute exacerbation of chronic bronchitis: 400 mg twice daily for 10 days
  • Pharyngotonsillitis and acute sinusitis: 200 mg twice daily for 10 days
  • Uncomplicated skin and soft structure infections: 200 mg twice daily for 10 days

Interaction

  • Antacids and Famotidine (intravenous administration) reduce oral absorption
  • Co-administration of probenecid increases plasma exposure
  • No effect on the pharmacokinetics of ethinyl estradiol

Contraindications

  • Known allergy to the cephalosporin class of antibiotics or any of its components
  • Contains sodium caseinate, a milk protein
  • Not recommended for patients with milk protein hypersensitivity

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women

Precautions & Warnings

  • Not recommended for prolonged antibiotic treatment
  • Unlikely to provide benefit without proven bacterial infection
  • Increases risk of drug-resistant bacteria

Use in Special Populations

  • Not recommended for pediatric patients less than 12 years of age
  • No dose adjustments necessary in geriatric patients
  • Dose adjustments based on renal function
  • No dose adjustments necessary for mild to moderate hepatic impairment

Overdose Effects

  • Adverse effects include nausea, vomiting, epigastric distress, diarrhea, and convulsions
  • Potential aid from hemodialysis
