Pharmacology
- Contains Amlodipine
- Contains Bisoprolol Fumarate
- Amlodipine is a dihydropyridine calcium antagonist
- Amlodipine inhibits calcium ion influx into cells
- Amlodipine reduces peripheral vascular resistance
- Bisoprolol Fumarate is a beta1-selective adrenoceptor blocking agent
- Bisoprolol Fumarate lacks intrinsic sympathomimetic activity
- Bisoprolol Fumarate extends beta1-selectivity beyond therapeutic dose range
Dosage & Administration
- One tablet once daily
- Recommended for patients with adequately controlled blood pressure
Interaction
- Combinations not recommended: Calcium antagonists of the verapamil and diltiazem type
- Combinations to be used with caution: Strong or moderate CYP3A4 inhibitors
- Combinations to be considered: Mefloquine
- Combinations to be used with caution: NSAIDs
Contraindications
- Acute heart failure
- Obstruction of the outflow tract of the left ventricle
- Cardiogenic shock
- Second or third degree AV block
- Sick sinus syndrome
- Severe bronchial asthma
- Untreated phaeochromocytoma
- Hypersensitivity to bisoprolol, amlodipine, or excipients
Side Effects
- Common: Dizziness
- Uncommon: Insomnia, mood changes
- Rare: Allergic reactions, nightmares
Pregnancy & Lactation
- Not recommended
Precautions & Warnings
- Increased risk of further deterioration of ventricular pump function in heart failure patients
- Caution in patients with impaired hepatic function
- Beta-blockers should be avoided in patients with obstructive airways diseases
Use in Special Populations
- Geriatric use: Usual doses can be administered to elderly with caution when increasing the dose
- Pediatric use: Not established for children and adolescents below 18 years
- Patients with Liver disease: Caution with hepatic impairment
- Patients with Kidney disease: No dosage adjustment for mild to moderate renal impairment
Overdose Effects
- Most common signs with beta-blocker overdose: Bradycardia, hypotension
- Signs of amlodipine overdose: Excessive peripheral vasodilation
