Indications
- Relief of symptoms in adults with acute and recurrent diabetic gastroparesis
- Treatment for 4 to 12 weeks of symptomatic, documented gastroesophageal reflux in adults who fail to respond to conventional therapy
- Prevention of nausea and vomiting associated with emetogenic cancer chemotherapy
- Facilitation of small bowel intubation in adults and pediatric patients
- Stimulation of gastric emptying and intestinal transit of barium during radiological examination
Pharmacology
Blocks dopamine and serotonin receptors, enhances motility and gastric emptying, increases lower esophageal sphincter tone
Dosage & Administration
- Recommended dosage for adults less than 65 years of age is 1 spray (15 mg) in one nostril, 30 minutes before each meal and at bedtime
- For gastroesophageal reflux, administer 10 to 15 mg, 30 minutes before each meal and at bedtime
- For diabetic gastroparesis, administer 10 mg, 30 minutes before each meal and at bedtime
- Intravenous infusions for prevention of nausea and vomiting associated with emetogenic cancer chemotherapy
Contraindications
- History of tardive dyskinesia (TD) or a dystonic reaction to metoclopramide
- Stimulation of gastrointestinal motility might be dangerous (e.g., in the presence of gastrointestinal hemorrhage, mechanical obstruction, or perforation)
- In patients with pheochromocytoma or other catecholamine-releasing paragangliomas
Side Effects
- Most common adverse reactions include dysgeusia, headache, and fatigue
Pregnancy & Lactation
- Pregnancy Category B
- Excreted in human milk, caution in nursing mothers
Precautions & Warnings
- Avoid concomitant use of other drugs known to cause TD/EPS/NMS
- Avoid use in patients with Parkinson's Disease
- Discontinue if symptoms of TD/EPS/NMS occur
- Avoid use in patients with depression and suicidal ideation/suicide
- Exercise caution in nursing mothers
