Terlyx 1 mg/8.5 ml (IV Injection)

Indications

• Short term management of bleeding oesophageal varices
• Emergency treatment of type 1 hepatorenal syndrome

Dosage & Administration

• Initial i.v. injection of 2 mg terlipressin acetate every 4 hours
• Maintenance dose adjustment based on patient's weight
• Standard duration of treatment is 10 days for type 1 hepatorenal syndrome

Interaction

• Increased hypotensive effect of non-selective beta-blockers
• Caution with concomitant medications that can prolong QT interval
• Extreme caution in patients with medications causing hypokalaemia or hypomagnesemia

Contraindications

• Hypersensitivity to active substance or excipients
• Contraindicated in pregnancy

• Increased blood pressure
• Abdominal pain
• Nausea
• Diarrhoea
• Headache

Pregnancy & Lactation

• Contraindicated during pregnancy
• Potential excretion in human breast milk
• Risk to the suckling child cannot be excluded

Precautions & Warnings

• Regular monitoring of blood pressure, ECG, heart rate, and serum levels of sodium and potassium
• Caution in patients with cardiac, pulmonary, and vascular disease
• Injection site reaction avoidance
• Avoid use in septic shock with low cardiac output
• Torsade de pointes caution
• Examine acute functional renal failure prior to treatment
• Exercise caution in paediatric and elderly patients
• Sodium content equivalent to 1.5% of recommended maximum daily intake

Use in Special Populations

Overdose Effects

• Avoid exceeding the recommended dose
• Control elevated blood pressure with clonidine i.v.
• Treat bradycardia with atropine
