  • Epistaxis
  • Prophylaxis and antihemorrhagic therapy during maxillofacial operations
  • Tooth extractions
  • To promote the formation of a fibrin capsule to wall off and inhibit the growth of ovarian tumors
  • To cause regression of ascites secondary to carcinoma
  • To reduce bleeding during surgical interventions

Pharmacology

  • Tranexamic acid (trans-4 aminomethyl-cyclohexanecarboxylic acid)
  • Strong antifibrinolytic action
  • Inhibition of plasminogen activation
  • Well absorbed by oral route
  • Effect seen 15-30 minutes after administration
  • Excreted mainly by renal route
  • Low acute and chronic toxicity
  • No interference with clotting processes
  • No tendency to thrombophilia

Dosage & Administration

  • Usual dose for adults: 500-1000 mg 3 times daily
  • Mean recommended daily doses for prophylaxis: 0.5-1 gm orally, 500 mg by the parenteral route
  • Therapy of hemorrhagic manifestations: oral dose increases to 1-3 gm given in divided doses
  • Usual dose for children for prophylaxis: 5-10 mg per kg of body weight orally daily in divided doses
  • For therapeutic purposes in children: Oral doses are doubled, intravenous and intramuscular treatment initiated with 10 mg/kg

Interaction

  • Incompatible with solutions containing penicillins
  • Thrombolytic drugs like Streptokinase & Urokinase antagonize the antifibrinolytic action
  • Increased potential for thrombus formation with concomitant administration of estrogen containing drugs
  • Direct admixture with whole blood to be avoided during transfusion

Contraindications

  • Known individual hypersensitivity to the product
  • Thromboembolic disease
  • Arterial and venous thrombosis
  • Endocavitary hemorrhages
  • Serious kidney failure

  • Infrequent cases of sense of fatigue
  • Conjunctival irritation
  • Nasal blockage
  • Itching
  • Skin reddening
  • Exanthems
  • Nausea
  • Diarrhea
  • Gastric pyrosis
  • Rare cases of postural hypotension

Pregnancy & Lactation

  • Tranexamic Acid should not be administered during known and presumed pregnancy
  • Passes into breast milk to a concentration of approximately one hundredth of the concentration in the maternal blood

Precautions & Warnings

  • Use in cases where there is hyperfibrinolysis
  • Prophylactic treatment to begin 24 hours before the operation
  • Therapy of hemorrhages to be prolonged for at least 24 hours after manifestations have disappeared
  • Reduce doses in cases of isolated hematuria
  • Avoid use in serious renal insufficiency or anuric syndromes
  • Particular care required in cardiopathic and hepatopathic subjects
