Indications

  • Prevention of rickets, osteomalacia, and osteoporosis
  • Promotes calcium and phosphate absorption
  • Boosts immunity
  • Prevents pre-eclampsia during pregnancy
  • Essential for infant growth

Pharmacology

  • Active form: Calcitriol
  • Binds to Vitamin D receptors (VDRs)
  • Fat soluble with a half-life of about 50 days
  • Metabolized to 25-hydroxy Vitamin D3
  • Increased absorption of calcium

Dosage & Administration

  • Adult dosage for Vitamin D3 deficiency: 40000 IU once weekly for 7 weeks
  • Adult maintenance therapy dosage: 1400-2000 IU/day
  • Children's dosage for Vitamin D3 deficiency: 20000 IU once every 2 weeks for 6 weeks
  • Dosage range for film-coated tablet: 1000-2000 IU daily
  • Dosage range for oroflash or chewable tablets: 1000-2000 IU daily
  • Injection dosage varies by age and condition

Interaction

  • Many drugs may cause Vitamin D3 deficiencies
  • Drugs include: Magnesium containing antacids, Digoxin, Thiazide diuretics, Cholestyramine, Cholistipol, Phenytoin, Phenobarbital, Orlistat, Mineral oil, Corticosteroids (e.g., Prednisolone)

Contraindications

  • Contraindicated in diseases associated with hypercalcemia
  • Contraindicated in hypersensitivity to Vitamin D3
  • Contraindicated if evidence of Vitamin D3 toxicity

Side Effects

  • Symptoms may include anorexia, nausea & vomiting, diarrhea, constipation, weight loss, polyuria, sweating, headache, thirst, vertigo, increased calcium and phosphate concentrations in plasma and urine

Pregnancy & Lactation

  • Safe use up to 4000 IU during pregnancy
  • Excreted in breast milk
  • Additional vitamin D3 for a breast-fed child should be considered carefully

Precautions & Warnings

  • Plasma-calcium concentration should be monitored at intervals
  • Consult a health care practitioner before supplementing with Vitamin D3 in certain conditions

Use in Special Populations

  • Safety & efficacy in children under 12 years not established
