Pharmacology

  • Synthetic glucocorticoid corticosteroid
  • Anti-inflammatory action
  • Practically insoluble in water
  • Very soluble in alcohol

Dosage and administration

  • Initial dose may vary from 2.5 mg to 100 mg per day
  • Suggested initial intramuscular dose is 60 mg
  • Strict aseptic technique is mandatory for administration
  • For systemic therapy, injection should be deeply into the gluteal muscle
  • For intra-articular administration, a single local injection is often sufficient

Contraindications

  • Hypersensitivity to any components of the product
  • Contraindicated for idiopathic thrombocytopenic purpura

Interaction

  • Aminoglutethimide may lead to loss of corticosteroid-induced adrenal suppression
  • Concurrent use of corticosteroids and potassium-depleting agents should be observed closely for hypokalemia
  • Macrolide antibiotics may cause a significant decrease in corticosteroid clearance
  • Concomitant use of anticholinesterase agents and corticosteroids may produce severe weakness in patients with myasthenia gravis

Side effects

  • Adverse reactions may include allergic reactions, cardiovascular effects, dermatologic effects, endocrine disturbances, fluid and electrolyte disturbances, gastrointestinal effects, metabolic effects, musculoskeletal effects, neurologic/psychiatric effects, ophthalmic effects

Pregnancy and lactation

  • Pregnancy category C
  • Systemically administered corticosteroids appear in human milk and could suppress growth or interfere with endogenous corticosteroid production
  • Caution should be exercised when administered to a nursing woman

Precautions and warnings

  • Sensitive to heat, should not be autoclaved for sterilization
  • The lowest possible dose should be used to control the condition
  • Corticosteroids should be used with caution in patients with cardiac, renal, gastrointestinal, and musculoskeletal conditions

Special populations

  • Pediatric use: Premature, low-birth-weight infants, and high dosages may be more likely to develop toxicity
  • Geriatric use: No overall differences in safety or effectiveness observed

Overdose effects

  • Treatment is by supportive and symptomatic therapy
  • For chronic overdosage, dosage may be reduced temporarily or alternate day treatment may be introduced
