Indications
- Bronchial asthma
- Bronchospasm
- Chronic obstructive pulmonary disease (COPD)
- Pulmonary disease with spastic bronchial component
Pharmacology
- Doxophylline is a novel bronchodilator
- Structurally differs from Theophylline due to the presence of a dioxolane group in position 7
- Selectively inhibits phosphodiesterase 4 thereby relaxes bronchial smooth muscle
- Reported to inhibit platelet activating factor (PAF) and generation of leukotriene production
Dosage & Administration
- 200 mg tablet two or three times daily for elderly
- 400 mg tablet two or three times daily for adults
- 10 ml syrup or 200 mg tablet two or three times daily for >12 years of age children
- 6-9 mg/kg body weight two times daily for 6-12 years of age children
- Doxophylline SR tablet to be taken once daily if required daily dose is 400 mg
Interaction
- Not to be administered with other xanthine preparations
- Limit consumption of beverages and food containing caffeine
- Exercise caution with ephedrine or other sympathomimetic drugs
- Concurrent administration with erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine and propanolol should be done carefully

- Known hypersensitivity to the drug or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- Lactation

Pregnancy & Lactation
- Effects during pregnancy should be evaluated carefully case by case on the basis of the risk-benefit ratio
- Contraindicated during lactation
- Does not interfere with ability to drive and use machines
Precautions & Warnings
- Age, congestive cardiac decompensation, chronic obstructive pulmonary disease, severe liver disease, concomitant infections may reduce hepatic clearance
- Monitoring of blood levels is recommended in cases of factors that influence the clearance of xanthine derivatives
- Caution in administering to patients with cardiac disease, hypertension, elderly, severe hypoxemia, hyperthyroidism, chronic corpulmonale, congestive heart failure, liver disease, peptic ulcer, and renal impairment
Overdose Effects
- Severe cardiac arrhythmias
- Tonic-clonic seizure
- Require discontinuation of the treatment
- After symptoms of toxicity subside, treatment may be resumed at lower doses
