Indications
- Chronic management of hyperuricemia in patients with gout
- Not recommended for the treatment of asymptomatic hyperuricemia
Pharmacology
- Non-purine, selective xanthine oxidase (XO) inhibitor
- Decreases serum uric acid level
- Inhibits xanthine oxidase responsible for uric acid production
Dosage & Administration
- Recommended starting dose: Febuxostat 40 mg tablet once daily
- Titration to 80 mg tablet and 120 mg tablet based on uric acid levels
- Dosage for Tumor Lysis Syndrome and Gout Flares
- No dose adjustment for renal impairment
- No dose adjustment for mild to moderate hepatic impairment
- Safety and effectiveness in pediatric patients under 18 years of age not established
Interaction
- Contraindicated with Mercaptopurine/Azathioprine
- No dose adjustment required with Rosiglitazone/CYP2C8 substrates
- Dependence on Uridine Glucuronosyl Transferase (UGT) enzymes for metabolism
- Possible interaction with glucuronidation inducers
- Can be co-administered with colchicine, indomethacin, hydrochlorothiazide, and warfarin
Contraindications
- Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Most commonly reported adverse reactions include gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and edema
- Rare serious hypersensitivity reactions observed
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
- Caution during breastfeeding
Precautions & Warnings
- Increase in gout flares during initiation of anti-hyperuricemic agents
- Higher rate of cardiovascular thromboembolic events observed
- Transaminase elevations observed in Floxtat-treated patients
Overdose Effects
- Studied in healthy subjects up to 300 mg daily for seven days without evidence of dose-limiting toxicities
