Description
- Prucalo is a medication indicated for symptomatic treatment of chronic constipation in adults in whom laxatives fail to provide adequate relief.
Dosage
- 2 mg once daily for adults
- Start with 1 mg for older people, can be increased to 2 mg if needed
- Not recommended for children and adolescents younger than 18 years
Mode of Action
- Acts as a selective stimulator of the 5-HT4 receptors
- Stimulates motility by interacting specifically with 5-HT4 receptors in the GI tract
- Causes release of acetylcholine and contraction of the muscle layer of the colon
Interaction
- Low interaction potential with therapeutic concentrations
- Weak substrate for P-glycoprotein (P-gp)
- Not expected to affect the CYP-mediated metabolism of co-medicated medicinal products
- Possible interaction with potent inhibitors of P-gp like ketoconazole, verapamil, cyclosporine A, and quinidine
Contraindications
- Hypersensitivity to the active substance or any of the excipients
- Renal impairment requiring dialysis
Side Effects
- Frequently reported adverse reactions include headache, abdominal pain, nausea, and diarrhea
- Adverse reactions occur predominantly at the start of therapy and usually disappear within a few days
Pregnancy & Lactation
- Not recommended during pregnancy
- Women of childbearing potential should use effective contraception during treatment
- Not recommended during breastfeeding
Precautions & Warnings
- Dose adjustment in severe renal impairment
- Caution in prescribing to patients with severe hepatic impairment
- Possible reduction in efficacy of oral contraceptives in severe diarrhea cases
Use in Special Populations
- Recommended lower dose for patients with severe renal impairment (GFR <30 ml/min/1.73 m^2)
- Start with lower dose for patients with severe hepatic impairment (Child-Pugh class C), no dose adjustment for mild to moderate impairment
Overdose Effects
- May result in symptoms like headache, nausea, and diarrhea
- No specific treatment available, manage symptomatically and supportive measures instituted as required
