Indications
- Symptomatic treatment of chronic constipation in adults
Pharmacology
- Stimulates motility by interacting with 5-HT4 receptors
- Acts as a selective stimulator of 5-HT4 receptors
- Causes release of acetylcholine, a major excitatory neurotransmitter in the GI tract
Dosage & Administration
- Adult dosage: 2 mg once daily with or without food
- Start with 1 mg once daily for older people
- Not recommended for children and adolescents younger than 18 years
Interaction
- Low interaction potential with CYP-mediated metabolism
- May have small interactions with potent inhibitors such as ketoconazole, verapamil, cyclosporine A, and quinidine
- No clinically relevant effects on the pharmacokinetics of certain drugs
Contraindications
- Hypersensitivity to the active substance or excipients
- Renal impairment requiring dialysis
Side Effects
- Most frequently reported adverse reactions: headache (17.8%) and gastrointestinal symptoms
- Majority of adverse events were mild to moderate in intensity
Pregnancy & Lactation
- Not recommended during pregnancy
- Effective contraception recommended for women of childbearing potential during treatment
- Not recommended during breastfeeding based on lack of human data
Precautions & Warnings
- Recommended dose adjustment in severe renal impairment
- Limited data in patients with severe hepatic impairment
- Possible reduced efficacy of oral contraceptives in case of severe diarrhea
Use in Special Populations
- Dose adjustment for patients with severe renal impairment
- Dose adjustment for patients with severe hepatic impairment
- No dose adjustment required for mild to moderate renal or hepatic impairment
Overdose Effects
- Symptoms may include headache, nausea, and diarrhea
