Indications
- Treatment of painful muscle spasms
- Associated with static and functional disorders of the spine (cervical and lumbar syndromes)
- Following surgery, e.g. for herniated intervertebral disc or osteoarthritis of the hip
- Treatment of spasticity due to neurological disorders
- Multiple sclerosis
- Chronic myelopathy
- Degenerative spinal cord diseases
- Cerebrovascular accidents
- Cerebral palsy
Pharmacology
- Centrally acting skeletal muscle relaxant
- Principal site of action is the spinal cord
- Stimulates presynaptic alpha2 receptors
- Inhibits release of excitatory amino acids
- Inhibits polysynaptic signal transmission at spinal interneuron level
- Moderate central analgesic effect
- Effective in acute painful muscle spasms and chronic spasticity
- Reduces resistance to passive movements
- Alleviates spasms and clonus
- May improve voluntary strength
- Antispastic activity
Dosage & Administration
- Narrow therapeutic index
- High inter-patient variability in plasma concentrations
- Initial dose of 2 mg three times daily
- Relief of painful muscle spasms: 2 to 4 mg three times daily
- Spasticity due to neurological disorders: initial daily dose not to exceed 6 mg
- Usual daily dose of 12 to 24 mg
- Maximum daily dose of 36 mg
- Discontinuation requires slow down titration
Interaction
- Concomitant administration with CYP1A2 inhibitors may increase plasma levels
- Concomitant administration with CYP1A2 inducers may decrease plasma levels
- Contraindicated with fluvoxamine or ciprofloxacin
- Caution with drugs known to prolong the QT interval
- Potential interactions with antihypertensives, rifampicin, cigarette smoke, alcohol, sedatives, and hypnotics
Contraindications
- Known hypersensitivity to tizanidine or excipients
- Severely impaired hepatic function
- Concomitant use with strong CYP1A2 inhibitors

- Caution with CYP inhibitors and drugs known to increase the QT interval
