Indications
- Relief from chronic heartburn symptoms and other symptoms associated with GERD

Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Enteric-coated pellet formulation
- Peak plasma levels occur at approximately 1.5 hours after oral administration
- 97% bound to plasma proteins
- Extensively metabolized in the liver by the cytochrome P450 enzyme system
- Plasma elimination half-life of approximately 1-1.5 hours
- Systemic bioavailability is approximately 90% after repeated once daily dosing
Dosage
- 20 mg or 40 mg Once Daily for 4-8 Weeks for healing of erosive esophagitis
- 20 mg Once Daily for maintenance of healing of erosive esophagitis
- 20 mg Once Daily for symptomatic GERD
- Triple Therapy for Helicobacter pylori eradication: Esomeprazole 40 mg Once Daily for 10 days, Amoxicillin 1000 mg Twice Daily for 10 days, Clarithromycin 500 mg Twice Daily for 10 days
- 20-80 mg once daily for Zollinger-Ellison Syndrome
- 20-40 mg once daily for Acid-related Dyspepsia for 2-4 weeks according to the response
- 20 mg once daily for 2-4 weeks for Duodenal ulcer and 20-40 mg once daily for 4-8 weeks for Gastric ulcer
- Dosage adjustment for pediatric and adult patients receiving IV Injection or IV infusion
- 80 mg for risk reduction of rebleeding of Gastric or Duodenal Ulcers following therapeutic endoscopy in adults
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral Suspension should be taken with water and stirred well before consumption
- IV Injection: Solution for injection prepared with 0.9% Sodium Chloride should be administered intravenously over a period of at least 3 minutes
- Specific instructions for administration through a nasogastric or gastric tube
Interaction
- No clinically relevant interactions with drugs metabolized by several CYP enzymes
- Potential interference with absorption of drugs where gastric pH is important for bioavailability
- Increased plasma levels of diazepam observed with co-administration
Contraindications
- Known hypersensitivity to any of the formulation
Side Effects
- Frequently occurring adverse events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Animal studies have revealed no teratogenic effects
- Excretion of esomeprazole in milk not studied, breastfeeding should be discontinued if essential
Precautions & Warnings
- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Instructions for patients with difficulty swallowing capsules
- Use of antacids while taking S-Ome
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety and efficacy observed between elderly and younger individuals
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- No alteration in pharmacokinetics expected in patients with renal impairment
Overdose Effects
- Acute toxicity signs include reduced motor activity, respiratory changes, tremor, and convulsions
- No specific antidote known, treatment should be symptomatic and supportive
Storage Conditions
- Store at a temperature not exceeding 30°C in a dry place
- Protect from light and moisture
- Keep out of reach of children
