Description
- Recombinant human erythropoietin (EPO)
- 165 amino acid sequence identical to that of human urinary EPO
- Apparent molecular weight of erythropoietin is about 30,400 daltons
Indications
- Treatment of anemia due to Chronic Kidney Disease (CKD) in patients on dialysis and not on dialysis
- Treatment of anemia due to Zidovudine in HIV-infected patients
- Reduction of allogeneic RBC transfusions in patients undergoing elective, noncardiac, nonvascular surgery
- Prevention of anemia of prematurity in infants with a birth weight of 750 to 1500 gm and gestational age of less than 34 weeks
Dosage
- Evaluation of iron stores and nutritional factors before and during treatment
- Monitoring hemoglobin levels at least weekly until stable, then monitor at least monthly
- Adjusting therapy based on hemoglobin rate of rise, rate of decline, ESA responsiveness, and hemoglobin variability
- Initiating treatment when hemoglobin level is less than 10 g/dL in CKD patients on dialysis
- Initiating treatment when hemoglobin level is less than 10 g/dL in CKD patients not on dialysis
- Starting dose recommendations for adult and pediatric patients
- Dose adjustment based on the response
Interaction
- Potentiation of Epoetin effect by simultaneous therapeutic administration of a haematinic agent such as ferrous sulphate
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- Patients who develop Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Adverse reactions in 5% of Epoetin treated patients in clinical studies
- Variety of side effects observed in different patient groups
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Reproduction toxicology observed in animal studies
- Caution advised for use in pregnancy and lactation
Overdose Effects
- Wide therapeutic margin of Epoetin
- Possible management with discontinuation or reduction of dosage and/or with phlebotomy
- Cases of severe hypertension observed following overdose with ESAs
