Description
- Pulmofib is indicated for the treatment of mild to moderate Idiopathic Pulmonary Fibrosis (IPF). Pirfenidone, the active ingredient, has anti-inflammatory, antioxidant, and anti-fibrotic properties. It is administered orally with food. Dose should be titrated to the recommended daily dose over a 14-days period. Dosage adjustments may be necessary for gastrointestinal events, photosensitivity reaction or rash, and hepatic function. It interacts with certain medicines and should be stored in a cool & dry place, protected from light & moisture.
Indications
- Treatment of mild to moderate Idiopathic Pulmonary Fibrosis (IPF) in adults
Pharmacology
- Novel agent with anti-inflammatory, antioxidant, and anti-fibrotic properties
- May improve lung function and reduce acute exacerbations in patients with IPF
- Mechanism of action involves the suppression of TGF-β1 and downregulation of pro-inflammatory cytokines
- Can inhibit influx of inflammatory cells and increased pulmonary vascular permeability induced by bleomycin
Dosage & Administration
- Orally administered with food
- Initial titration to recommended daily dose over 14 days
- Dosage adjustment for gastrointestinal events, photosensitivity reaction, and hepatic function
Interaction
- Moderate and strong inhibitors of CYP1A2 can alter the adverse reaction profile
- Food intake reduces the incidence of adverse reactions
Contraindications
- Hypersensitivity to the active substance or excipients
- Concomitant use of fluvoxamine
- Severe hepatic or renal impairment
Side Effects
- Common: Nausea, rash, abdominal pain, upper respiratory tract infection, diarrhea, fatigue, headache
- Rare: Photosensitivity reaction, decreased appetite, pruritus, asthenia, dysgeusia
Pregnancy & Lactation
- Preferable to avoid use during pregnancy
- Risk to the suckling child cannot be excluded during lactation
Precautions & Warnings
- Monitor liver enzymes, photosensitivity, and gastrointestinal disorders
- Avoid exposure to sunlight and sunlamps
Use in Special Populations
- Not known if safe and effective in children & adolescents
- Not recommended in severe hepatic impairment or end-stage renal disease
- Decreased exposure noted in smokers
Overdose Effects
- Limited clinical experience with overdose
- Adverse reactions consistent with most frequently reported adverse reactions
