Indications
- Hyperlipidemias of types IIA, IIB, III, IV & V
- Patients not responding to diet & other measures
Pharmacology
- Fibric acid derivative
- Rapid hydrolysis to fenofibric acid
- Reductions in total cholesterol & LDL cholesterol
- Increases in HDL & apo-proteins
- Reduces serum uric acid levels
- Enhanced absorption in micronised form
Dosage & Administration
- Initial dose for hypercholesterolemia/mixed hyperlipidemia: 200 mg per day
- Initial dose for hypertriglyceridemia: 67-200 mg per day
- Dose individualization as per patient response
- Should be given with meal for optimal bioavailability
Interaction
- Potentiation of anticoagulant effects of warfarin
- Improvement in glucose tolerance when administered with antidiabetic drug
- Increase in nephrotoxicity of cyclosporine
- Potential risk of rhabdomyolysis with HMG-CoA reductase inhibitors
- Well tolerated with low-dose statins
Contraindications
- Hypersensitivity to fenofibrate
- Severe renal or hepatic impairment
- Existing gall bladder disease
- Breastfeeding mothers
- Photosensitivity to ketoprofen
Side Effects
- Gastrointestinal effects (nausea, anorexia, gastric pain)
- Pruritus, urticaria
- Impotence, headache, dizziness, vertigo, fatigue, hair loss
- Myotoxicity
Pregnancy & Lactation
- Not recommended for pregnant women
Precautions & Warnings
- Special care needed in patients with renal disease
- Progressive increases in serum creatinine concentration may result in myotoxicity
- LFT recommended every 3 months for first year
Overdose Effects
- No specific treatment for overdose
- General supportive care
- Hemodialysis not recommended due to high plasma protein binding
