Pharmacology
Provides anti-inflammatory action by inhibiting Phospholipase A2 enzyme and decreases the release of certain chemicals important in the immune system.
Dosage & Administration
- Adults:
- For acute disorders: Up to 120 mg/day initially
- Maintenance doses usually within the range of 3-18 mg/day for most conditions
- Children:
- Juvenile chronic arthritis: Usual maintenance dose is between 0.25-1.0 mg/kg/day
- Nephrotic syndrome: Initial dose of usually 1.5 mg/kg/day followed by down titration according to clinical need
- Deflazacort withdrawal: In patients who have received more than physiological doses of systemic corticosteroids, withdrawal should not be abrupt
Interaction
Metabolized in the liver. Maintenance dose of Defcort may need to be increased if co-administered with liver enzyme inducers like rifampicin, carbamazepine, etc. Reduced maintenance dose possible with liver enzyme inhibitors
Contraindications
- Hypersensitivity to any of the ingredients
- Patients receiving live virus immunization

Pregnancy & Lactation
- Pregnancy: May increase the risk of intrauterine growth retardation if used repeatedly during pregnancy
- Nursing Mother: Doses of up to 50 mg daily unlikely to cause systemic effects in the infant. Infants of mothers taking higher doses may have a degree of adrenal suppression

- Hypothyroidism and cirrhosis
- Ocular herpes simplex
Use in Special Populations
- Hepatic Impairment: Blood levels may be increased, so careful monitoring and adjustment of the dose is necessary
- Renal Impairment: No special precautions necessary
- Elderly: Common adverse effects may have more serious consequences in old age
