Indications
- Indicated for hyperlipidemias of type IIa, IIb, III, IV, & V
- For patients who have not responded to diet & other measures
Pharmacology
- Fenofibrate is a fibric acid derivative
- Rapidly hydrolyzed to fenofibric acid
- Reductions in total cholesterol, LDL cholesterol, Apo-lipoprotein B
- Increases in HDL and apo-proteins apoAI apoAII
- Reduces serum uric acid levels
- Enhanced absorption with micronised form
Dosage & Administration
- Initial dose for hypercholesterolemia/mixed hyperlipidemia: 200mg/day
- Initial dose for hypertriglyceridemia: 67-200mg/day
- Dose should be individualized according to patient response
- Should be given with meal to optimize bioavailability
Interaction
- Potentiates anticoagulant effects of warfarin
- May improve glucose tolerance
- May increase nephrotoxicity of cyclosporine
- Cautions against use with HMG-CoA reductase inhibitors
Contraindications
- Hypersensitivity to Fenofibrate
- Severe renal or hepatic impairment
- Existing gall bladder disease
- Breastfeeding mothers
- Photosensitivity to ketoprofen
Side Effects
- Gastro-intestinal symptoms (nausea, anorexia, gastric pain)
- Pruritus, urticaria
- Impotence, headache, dizziness, vertigo, fatigue
- Hair loss, myotoxicity
Pregnancy & Lactation
- Not recommended for pregnant women
Precautions & Warnings
- Special care needed in patients with renal disease
- Liver function tests recommended every 3 months
- Discontinue if myotoxicity suspected
Overdose Effects
- No specific treatment for overdose
- General supportive care indicated
- Hemodialysis not recommended
