Indications
- Hyperlipidemias of type lla, llb, III, IV & V
- For patients who have not responded adequately to diet & other measures
Pharmacology
- Fenofibrate is a fibric acid derivative
- Rapidly hydrolyzed after oral ingestion to its active form - fenofibric acid
- Reduces total cholesterol, LDL cholesterol, Apo-lipoprotein B, Total triglycerides and VLDL
- Increases HDL and apo-proteins apoAI apoAII
- Reduces serum uric acid levels
Dosage & Administration
- Initial dose for hypercholesterolemia or mixed hyperlipidemia: 200 mg per day
- Initial dose for hypertriglyceridemia: 67 to 200 mg per day
- Dose should be individualized according to patient response
- Fenofibrate should be given with meal to optimize bioavailability
- As directed by the registered physician
Interaction
- Potentiates the anticoagulant effects of warfarin
- May improve glucose tolerance and have additive effect with antidiabetic drugs
- May increase the nephrotoxicity of cyclosporine
- Cautions against use with HMG-CoA reductase inhibitors
- Low-dose statins with Trigent appears to be well tolerated
Contraindications
- Hypersensitivity to Fenofibrate
- Severe renal or hepatic impairment
- Existing gall bladder disease
- Breast feeding mothers
- Photosensitivity to ketoprofen
Side Effects
- Gastro-intestinal discomfort (e.g. nausea, anorexia, gastric pain)
- Pruritus, urticaria
- Impotence
- Headache, dizziness, vertigo, fatigue, hair loss
- Myotoxicity
Pregnancy & Lactation
- Not recommended for pregnant women
Precautions & Warnings
- Special care needed in patients with renal disease
- Discontinue if myotoxicity suspected or creatinine kinase increases significantly
- Liver function tests recommended every 3 months for first year
Overdose Effects
- No specific treatment for overdose
- General supportive care indicated
- Monitoring of vital signs and clinical status recommended
- Hemodialysis should not be considered
