Pharmacology

Cefixime is a third generation semisynthetic cephalosporin antibiotic for oral administration, bactericidal against a broad spectrum of gram positive and gram negative bacteria by interfering in the synthesis of bacterial cell wall.

Interferes with bacterial cell wall synthesis

Absorption rate

Approximately 40% to 50%

Recommended course

7 to 14 days

Recommended dosage

  • Adults: 200-400 mg daily
  • Adults note: Single dose or in two divided doses
  • Elderly: Same dose as recommended for adults
  • Children over 10: 200-400 mg daily
  • Children under 10: 8 mg/kg/day

Recommended dosage by age

  • 6 months to 1 year: 3.75 ml daily
  • 1 to 4 years: 5 ml daily
  • 5 to 10 years: 10 ml daily
  • Typhoid children: 5 mg/kg body weight twice daily for 10-14 days

Renal impairment dosage

In patients with creatinine clearances of less than 20 ml/min, a dose of 200 mg once daily should not be exceeded

Interaction

  • Increases in prothrombin times have been noted with anticoagulant therapy

  • Known hypersensitivity to cephalosporin antibiotics

Pregnancy and lactation

Use during pregnancy only if clearly needed. Caution should be exercised when administered to a nursing woman

Precautions and warnings

  • Allergies: Given with caution to patients who have shown hypersensitivity to other drugs
  • Penicillin sensitive patients: Given with caution due to partial cross-allergenicity between penicillins and cephalosporins
  • Renal function: Given with caution in patients with markedly impaired renal function
  • Antibiotic associated diarrhoea: Alters the normal flora of the colon and may permit overgrowth of clostridia

Overdose effects

Gastric Lavage may be indicated; otherwise, no specific antidote exists. Adverse reactions in small numbers of healthy adult volunteers receiving single doses up to 2 g of Prioxim did not differ from the profile seen in patients treated at the recommended doses
