Indications
- treatment of partial-onset seizures
- monotherapy for adults and adolescents
- adjunctive therapy for adults, adolescents, children, and infants

Description
- unknown precise mechanism of antiepileptic effect
- minimal activity in submaximal stimulation
- protection against secondarily generalized activity
- inhibition of burst firing without affecting normal neuronal excitability
Dosage
- recommended starting dose of 250 mg twice daily for adults and adolescents from 16 years of age
- initial therapeutic dose of 500 mg twice daily
- maximum dose of 1500 mg twice daily for adults and adolescents
- adapted oral solution for infants and children under the age of 6 years
Administration
- tablet form for oral administration
- to be taken with sufficient liquid
- administered in two equally divided doses
Interaction
- no influence on serum concentrations of existing antiepileptic medicinal products
- interaction with probenecid and methotrexate
- reports of decreased efficacy when concomitantly administered with macrogol
- no alteration in absorption with food
- slightly reduced absorption rate by food
Contraindications
- hypersensitivity to the active substance or other pyrrolidone derivatives or excipients
Side Effects
- common adverse reactions in adult and pediatric patients
- incidence ≥ 5% more than placebo
- includes somnolence, asthenia, infection, dizziness, fatigue, aggression, nasal congestion, decreased appetite, irritability
Pregnancy & Lactation
- not recommended during pregnancy and lactation
- excreted in human breast milk
- impact on fertility is unknown
Precautions & Warnings
- dose adjustment for renal impairment
- rare cases of decreased blood cell counts
- association with acute kidney injury
- monitoring for signs of depression and suicidal ideation
- tablet formulation not adapted for use in children under 6 years
- impact on growth and puberty needs further study
Use in Special Populations
- dose adjustment in elderly and patients with compromised renal or hepatic function
Overdose Effects
- observed effects include somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma

- may include haemodialysis
