Indications
- Monotherapy in the treatment of partial-onset seizures in adults and adolescents
- Adjunctive therapy for partial-onset seizures in adults, adolescents, children, and infants
- Treatment of myoclonic seizures in adults and adolescents
- Treatment of primary generalized tonic-clonic seizures in adults and adolescents
Description
- Mechanism of action is unknown
- Minimal activity in submaximal stimulation and threshold tests
- Inhibition of burst firing without affecting normal neuronal excitability
Dosage
- Recommended starting dose for adults and adolescents from 16 years of age is 250 mg twice daily
- Maximum dose is 1500 mg twice daily
- Initial therapeutic dose for adults (≥18 years) and adolescents (12 to 17 years) weighing 50 kg or more is 500 mg twice daily
- Safety and efficacy of levetiracetam in children and adolescents below 16 years as monotherapy treatment have not been established
Administration
- Must be taken orally with sufficient liquid
- Administered in two equally divided doses
Interaction
- Did not influence the serum concentrations of existing antiepileptic medicinal products
- Probenecid inhibits the renal clearance of the primary metabolite but not of Litam
- Concomitant administration of Litam and methotrexate has been reported to decrease methotrexate clearance
- Macrogol should not be taken orally for one hour before and after taking Litam
- No alteration in absorption by food, but the rate of absorption was slightly reduced
Contraindications
- Hypersensitivity to the active substance or other pyrrolidone derivatives or to any of the excipients
Side Effects
- Most common adverse reactions include somnolence, asthenia, infection, dizziness in adult patients
- Fatigue, aggression, nasal congestion, decreased appetite, irritability in pediatric patients
Pregnancy & Lactation
- Not recommended during pregnancy and breastfeeding unless clinically necessary
- Excreted in human breast milk
Precautions & Warnings
- Administration to patients with renal impairment may require dose adjustment
- Associated with acute kidney injury
- Rare cases of decreased blood cell counts have been described
- Suicidal ideation and behavior have been reported in patients treated with anti-epileptic agents
- Tablet formulation is not adapted for use in infants and children under the age of 6 years
- Effects on learning, intelligence, growth, endocrine function, and childbearing potential in children remain unknown
Use in Special Populations
- Adjustment of the dose is recommended in elderly patients with compromised renal function
- Daily dose must be individualized according to renal function
- No dose adjustment is needed in patients with mild to moderate hepatic impairment
- A 50% reduction of the daily maintenance dose is recommended in patients with severe hepatic impairment
Overdose Effects
- Somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma were observed with Litam overdoses
- No specific antidote for Litam
- Symptomatic treatment may include haemodialysis
