Indications
- Treatment of hypertension
- Can be used alone or in combination with other antihypertensive agents
Pharmacology
- Angiotensin II formation from angiotensin I
- Vasoconstrictor effects of olmesartan
- Selective blocking of AT 1 receptor in vascular smooth muscle
- AT 1 receptor affinity over 12500-fold greater than AT 2 receptor
- Does not inhibit ACE (kininase II) or affect bradykinin response
- Inhibition of angiotensin II receptor negative regulatory feedback on renin secretion
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- 40 mg for further reduction in blood pressure after 2 weeks
- No advantage of twice-daily dosing
- No initial dosage adjustment recommended for elderly patients or those with moderate to marked renal or hepatic impairment
- Consideration for use of lower starting dose in patients with possible depletion of intravascular volume
- May be administered with or without food
Interaction
- No significant drug interactions with digoxin or warfarin reported
- Bioavailability not significantly altered by co-administration of antacids
- Not metabolized by cytochrome P450 system
- No effect on P450 enzymes
- Potential interaction with Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) including selective Cyclooxygenase-2 Inhibitors (COX-2 Inhibitors) leading to deterioration of renal function
- Antihypertensive effect may be attenuated by NSAIDs including selective COX-2 inhibitors
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well-tolerated with adverse reaction incidence similar to placebo
- Dizziness as the only adverse reaction occurring in more than 1% of patients
- Other adverse reactions occurring at an incidence of more than 1%
- No dose-related overall frequency of adverse reactions
Pregnancy & Lactation
- Discontinue use when pregnancy is detected
- Potential excretion in human milk with unknown effects on nursing infant
Precautions & Warnings
- Potential changes in renal function
- Association with oliguria and/or progressive azotemia in susceptible individuals
- Similar effects to those observed with angiotensin-converting enzyme inhibitors and angiotensin receptor antagonists
Overdose Effects
- No experience of overdose with Olmefast
- Likely effects include hypotension and tachycardia
- Bradycardia possible with parasympathetic (vagal) stimulation
- Gastric lavage or induction of emesis may be considered for recent intakes
- Clinically significant hypotension requires active cardiovascular system support
