Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Blocks the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
- Does not inhibit ACE (kininase II)
- Does not affect the response to bradykinin
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- May be increased to 40 mg for further reduction in blood pressure
- No advantage in twice-daily dosing
- No initial dosage adjustment recommended for elderly patients
- No initial dosage adjustment recommended for patients with moderate to marked renal impairment
- No initial dosage adjustment recommended for patients with moderate to marked hepatic dysfunction
- May be administered with or without food
Interaction
- No significant drug interactions with digoxin or warfarin
- Bioavailability not significantly altered by co-administration of antacids
- Not metabolized by the cytochrome P450 system
- No effects on P450 enzymes
Contraindications
- Contraindicated in patients hypersensitive to any component
Side Effects
- Well tolerated with similar incidence of adverse reactions to placebo
- Adverse reactions not dose related
- No differences in adverse reactions based on gender, age, and race
- Withdrawal rate due to adverse reactions: 2.4% for Olmesartan and 2.7% for control
- Common adverse reactions: dizziness, back pain, diarrhea, headache, hyperglycemia, etc.
Pregnancy & Lactation
- Discontinue use when pregnancy is detected
- Drugs that act directly on the renin-angiotensin system can cause injury to the developing fetus
- Not known whether excreted in human milk
- Decide whether to discontinue nursing or discontinue the drug based on importance to the mother
Precautions & Warnings
- Inhibiting the renin-angiotensin-aldosterone system may cause changes in renal function
- Associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- No experience of overdose reported
- Likely effects: hypotension, tachycardia, bradycardia if parasympathetic stimulation occurs
- Recommended actions for recent intake: gastric lavage or induction of emesis
