Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocker of hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasting up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • 20 mg once daily for 4 weeks in duodenal ulceration
  • 40 mg daily in severe or recurrent cases of gastric ulceration
  • Maintenance dose for recurrent duodenal ulcer, 20 mg once daily
  • 40 mg once daily for 4 weeks in gastro-esophageal reflux disease
  • 10-20 mg daily for long-term management of acid reflux disease
  • 10-20 mg once daily for 2-4 weeks in acid-related dyspepsia
  • 40 mg on the preceding evening and 2-6 hours before surgery for prophylaxis of acid aspiration
  • Initially 60 mg once daily for Zollinger-Ellison syndrome
  • Omeprazole recommended at a dose of 20 mg twice daily for Helicobacter pylori eradication regimen in peptic ulcer disease
  • Pediatric use: 10-20 mg once daily for 4-12 weeks (if body-weight 10-20 kg), 20-40 mg once daily for 4-12 weeks (if body-weight over 20 kg)

Administration

  • Intravenous administration only
  • IV injection should be given as a slow intravenous injection
  • IV infusion should be given over a period of 20-30 minutes or more
  • Reconstituted solution should be used within 4 hours (IV injection) or 12 hours (IV infusion)
  • Reconstituted solution should not be mixed or co-administered with any other drug

Interaction

  • Reduced absorption of ketoconazole during treatment
  • Metabolized in the liver through cytochrome P450
  • Possible delay in the elimination of diazepam, phenytoin, and warfarin
  • Monitoring of patients receiving warfarin or phenytoin recommended
  • Increased plasma concentrations of Omeprazole and clarithromycin during concomitant administration

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspected gastric ulcer with possibility of malignancy

  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, and erythema multiforme
  • Diarrhoea
  • Headache
  • Constipation
  • Nausea/vomiting
  • Dry mouth
  • Paraesthesia
  • Dizziness
  • Somnolence
  • Reversible mental confusion, agitation, depression, and hallucinations
  • Arthritic and myalgic symptoms
  • Blurred vision
  • Taste disturbance
  • Peripheral edema
  • Increased sweating
  • Agranulocytosis
  • Pancytopenia
  • Hepatic failure

Pregnancy & Lactation

  • No adverse effects on pregnancy or on the health of the fetus/newborn child based on epidemiological studies
  • No information available on the passage of omeprazole into breast milk or its effects on the neonate
  • Discontinuation of breastfeeding if the use of omeprazole is considered essential

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and omeprazole
  • Possible association of PPI therapy with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in patients treated long-term with omeprazole
  • Concomitant use of PPIs with methotrexate may lead to methotrexate toxicities
