Indications

• Treatment of type 2 diabetes mellitus in adults
• Improves glycaemic control
• Can be used as monotherapy
• Can be used in combination therapy
• For patients inadequately controlled by diet and exercise
• For patients for whom metformin is inappropriate
• For patients with renal impairment

Pharmacology

• Linagliptin is an inhibitor of DPP-4 enzyme
• Increases concentrations of active incretin hormones
• Stimulates insulin release from pancreatic beta cells
• Decreases glucagon secretion from pancreatic alpha cells
• Improves glycemic control in type 2 diabetes mellitus

Dosage & Administration

• Linagliptin 5 mg once daily
• Can be taken with or without a meal
• No dose adjustment required for renal impairment
• Maintain dose when added to metformin
• Consider lower dose of sulphonylurea in combination therapy

Interaction

• Weak competitive and mechanism-based inhibitor of CYP3A4
• Low risk for clinically meaningful interactions
• No clinically relevant effects on pharmacokinetics of other medications

Contraindications

• Hypersensitivity to active substance or excipients

Side Effects

• Hypoglycaemia
• Nasopharyngitis
• Cough
• Pancreatitis
• Increased risk in combination with metformin and sulfonylurea

Pregnancy & Lactation

• Pregnancy category B
• Use only if clearly needed during pregnancy
• Unknown if Linagliptin passes into breast milk

• Hypersensitivity to active substance or excipients
