Indications

  • Improving glycemic control in adults with type 2 diabetes mellitus as an adjunct to diet and exercise

Pharmacology

  • Inhibitor of DPP-4 enzyme
  • Increases concentrations of active incretin hormones
  • Stimulates insulin release from pancreatic beta cells in a glucose-dependent manner
  • Decreases glucagon secretion from pancreatic alpha cells
  • Lowers basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerability
  • Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
  • Dose escalation should be gradual
  • Recommended starting dose based on patient's current treatment

Interaction

  • Potential interaction with cationic drugs
  • Risk of metabolic acidosis with carbonic anhydrase inhibitors
  • Potential reduction in efficacy when administered with inducers of P-glycoprotein and CYP3A4 enzymes
  • Careful observation for glycemic control when combined with specific drugs

Contraindications

  • Contraindicated in patients with renal impairment
  • Contraindicated in acute or chronic metabolic acidosis (diabetic ketoacidosis)
  • Contraindicated in hypersensitivity to Linagliptin or Metformin Hydrochloride

Side Effects

  • Most common side effects are nasopharyngitis and diarrhea
  • Increased risk of hypoglycemia when used with sulfonylureas

Pregnancy & Lactation

  • Should be used during pregnancy only if clearly needed
  • Caution when administered to a lactating mother

Precautions & Warnings

  • Discontinue immediately in cases of lactic acidosis
  • Temporary discontinuation in specific medical procedures
  • Monitoring of Vitamin B12 levels
