Indications
- Reduces total cholesterol in patients with familial hypercholesterolaemia
- Reduces triglycerides levels in patients with mixed dyslipidemia
- Treats hypertriglyceridaemia and dysbetalipoproteinaemia
- Reduces cardiac ischaemic events in patients with coronary artery disease
- Reduces cholesterol concentrations in patients with diabetes mellitus or renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors for enhanced uptake and catabolism of LDL
- Rapidly absorbed after oral administration
- Extensively metabolized to active derivatives
- Eliminated primarily in bile
- Half-life of inhibitory activity for HMG-CoA reductase
Dosage & Administration
- Initial dose is usually 10 mg once daily for adults
- Maximum dosage can be increased at intervals of at least 4 weeks
- Dosage range is 10 to 80 mg once daily
Interaction
- Increased risk of myopathy with concurrent administration of certain drugs
- Plasma concentrations affected by coadministration with antacid, colestipol, digoxin, erythromycin, oral contraceptives, or warfarin
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Precautions & Warnings
- Liver function tests should be performed before initiation of treatment
- Use with caution in patients with substantial alcohol consumption or history of liver disease
- Discontinue therapy if markedly elevated CPK levels occur or myopathy is diagnosed or suspected
Use in Special Populations
- Plasma concentrations are higher in healthy elderly subjects
- No clinically significant difference in LDL-C reduction between men and women
- Renal disease has no influence on plasma concentrations
- Hemodialysis not expected to significantly enhance drug clearance
- Markedly increased plasma concentrations in patients with chronic alcoholic liver disease
