Description
- Used to treat excessive postoperative bleeding
- Can be administered orally or intravenously
- Can reduce blood loss in patients undergoing coronary artery bypass grafting
- Can be used to treat overdose and/or toxic effects of thrombolytic pharmacologic agents
Indications
- Treatment of excessive postoperative bleeding
- Oral and intravenous administration
- Reduction of blood loss in coronary artery bypass grafting
- Treatment of thrombolytic pharmacologic agent overdose/toxic effects
Pharmacology
- Exerts fibrinolysis-inhibitory effects via inhibition of plasminogen activators and antiplasmin activity
Dosage & Administration
- Dosage adjustment according to severity of hemorrhagic event
- Rapid clearance by renal route necessitating repeated administration every 4-6 hours
- Average daily dose of 8-16 gm
- Equally effective by mouth and intravenous injection
- Administration continued until complete cessation of all symptoms
Contraindications
- Not to be used when evidence of active intravascular clotting process
- Caution needed in distinguishing primary fibrinolysis from disseminated intravascular coagulation (DIC) before administration
- Various tests to differentiate primary fibrinolysis from DIC
- Should not be used in the presence of DIC without concomitant heparin
Side Effects
- Mild muscle pain or weakness
- Headache
- Nausea, vomiting, stomach pain, diarrhea
- Decreased amount of semen in men
- Stuffy nose, watery eyes
- Vision problems, ringing in ears
- Mild skin rash
Pregnancy & Lactation
- Pregnancy Category C
- No animal reproduction studies conducted
- Uncertain if it can cause fetal harm or affect reproduction capacity
- Should be given to a pregnant woman only if clearly needed
Precautions & Warnings
- Safety and efficacy not established in patients younger than 18 years
