Indications
- Hypertension
- Renal Protection in Type-2 Diabetic Patients with Proteinuria
Pharmacology
- Non-peptide orally active angiotensin II receptor blocker
- Binds to the AT1 receptor
- Reduces vasoconstriction
- Reduces release of aldosterone
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- 25 mg twice daily if 50 mg is inadequate
- 25 mg once daily for intravascular volume-depleted patients
- Dosage ranges from 25 mg to 100 mg
- No initial dosage adjustment for patients upto 75 years
- Lower starting dose of 25 mg once daily for patients over 75 years
Interaction
- Rifampicin and fluconazole reduce levels of active metabolite
- Potentiation with concomitant use of hydrochlorothiazide
- Increases in serum potassium with potassium-sparing diuretics
- Attenuation by indomethacin
- Increased risk of renal impairment with certain drug combinations
Contraindications
- Contraindicated in pregnant women
- Contraindicated in patients hypersensitive to any component
- Not to be administered with Aliskiren in diabetic patients

Pregnancy & Lactation
- Pregnancy Category D
- Risk to fetus in second or third trimesters

Precautions & Warnings
- Reduction of fetal renal function and increase in morbidity and death during second and third trimesters
- Symptomatic hypotension in intravascularly volume-depleted patients
- Increased plasma concentration in cirrhotic patients
- Changes in renal function including renal failure in renal impaired patients
Use in Special Populations
- No initial dosage adjustment in patients with mild renal impairment
- Lower starting dose for moderate to severe renal impairment or patients on dialysis
- A lower dose should be considered for patients with a history of hepatic impairment
- Contraindicated in pregnancy and hypersensitivity to losartan
- Increased plasma concentrations in cirrhotic patients
- Increased blood urea and serum creatinine in patients with bilateral renal artery stenosis
- Reversible changes in renal function upon discontinuation
