Indications

  • Treatment of type 2 diabetes mellitus
  • Management of type 2 diabetes in overweight patients
  • Use in adults as monotherapy or in combination with other oral antidiabetic agents or with insulin
  • Use in children from 10 years of age and adolescents
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients
  • First-line therapy after diet failure

Pharmacology

  • Oral antihyperglycemic drug
  • Lowers basal and postprandial plasma glucose
  • Different mechanism of action from sulfonylureas
  • Does not produce hypoglycemia
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity
  • Increases peripheral glucose uptake and utilization

Contraindications

  • Breastfeeding not recommended during treatment

Precautions & Warnings

  • Substantial excretion by the kidney
  • Risk of D-Met accumulation and lactic acidosis
  • Lowering of vitamin B12 level
  • Increased risk of hypoglycemia
  • Regular renal function monitoring
  • Caution in conditions causing dehydration or serious infections
  • Annual estimation of Vitamin B-12 levels
  • Interaction with sulphonylurea and anticoagulants

Use in Special Populations

  • Adjusted dosage for elderly subjects
  • No effect on growth and puberty observed in controlled clinical studies
  • Recommended caution in prescribing to children aged between 10 and 12 years
  • Regular assessment of renal function
  • Exercise caution in situations where renal function may become impaired

Overdose Effects

  • No hypoglycemia observed with D-Met doses up to 85 gm
  • Lactic acidosis occurred in such circumstances
