Recommended Dosage
- Adult: Starting dose - 500mg twice a day or 850mg once a day, Children: Starting dose - 500mg twice a day
- Dosage increases in increments of 500mg or 850mg, up to a maximum of 2000mg per day
- Adult (Extended release tablet): Starting dose - 500mg once daily with the evening meal, Children: Not studied in children
Mode of Action
- Lowers both basal and postprandial plasma glucose, Decreases hepatic glucose production, Improves insulin sensitivity
Interaction
- D-Met and Furosemide co-administration
- Enhanced absorption of D-Met by Nifedipine
- Potential interaction with cationic drugs eliminated by renal tubular secretion
- Effects on Cimetidine pharmacokinetics
- Drugs that may produce hyperglycemia and impact glycemic control
Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure (GFR < 30 mL/min)
- Conditions with potential to alter renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption and peripheral neuropathy
- Taste disturbance, Encephalopathy
- Gastrointestinal disorders (nausea, vomiting, diarrhea, abdominal pain)
- Liver function test abnormalities or hepatitis
- Skin reactions (erythema, pruritus, urticaria)
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy linked to congenital abnormalities and perinatal mortality
- Metformin excretion into breast milk
- Decision on breastfeeding to consider potential risks
Precautions & Warnings
- Excretion by the kidney, risk of accumulation and lactic acidosis
- Lowering vitamin B12 level, increased risk of hypoglycemia
- Regular monitoring of renal function advised
- Caution in conditions causing dehydration, serious infections or trauma
- Annual estimation of Vitamin B12 levels recommended
- Stabilization of diabetic patients with D-Met and insulin in hospital

Overdose Effects
- Hypoglycemia not seen with doses up to 85 gm
- Lactic acidosis occurring with high doses
- Hemodialysis as the most effective method to remove lactate and D-Met
